Ventracare Homes, Inc. welcomes children who are medically ready to be discharged from the hospital but require some type of ongoing support beyond what they can receive at their family home. We encourage families to visit their children as often as they like. Families can easily access our transitional care home by public transportation, making visitation convenient. Additionally, our home is also near several pediatric medical facilities, including the Advocate Children’s Hospital – Oak Lawn.

Our home supports up to twelve children. Three nurses and one direct support professional staff at the home 24 hours a day, seven days a week. A medical director leads our team of skilled nurses, physical therapists, occupation therapists, speech-language pathology personnel, and social workers.

OUR SERVICES

Respite Care

Care for children who are under age 22, are medically complex, have a medical condition that requires care to be delivered by a nurse or a trained parent/caregiver, and who are admitted for no more than 14 days.

Respite care provides temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, or disabled family member.

Skilled Nursing Care
Skilled nursing care is provided by registered nurses or licensed practical nurses and home health aides.
Transitional Care
Transitional care is provided for up to 120 days to facilitate home placement or other appropriate settings and reunite families. Service is provided for medically fragile children, technology-dependent children, and children with special health care needs who are deemed clinically stable by a physician.
Medical Daycare Services
Medical daycare services are designed to optimize the development of each child’s independence while helping them attain their full potential. Services are provided for children who have skilled nursing and therapy needs that are provided by nurses, certified nursing assistants, early childhood educators, and therapists.
Weekend Camps
Weekend camps are planned programs for medically fragile children, technology-dependent children, or children with special health care needs that typically occur from Friday afternoon through Sunday evening.
Diagnostic studies
Diagnostic procedures are typically done in the home setting.
